Registration For New Students:
Registration must be done in person. Please call 781-956-4281 or e-mail srizkallah@comcast.net to schedule a placement meeting. Meetings must be scheduled and completed by June 1, 2012. At this meeting we will get to know the student better to ensure proper band placement. Please bring completed Registration Form , Code of Conduct Agreement and Media Release/Assumption of Risk Form with payment to your placement meeting.

ADDITIONAL REGISTRATION INFORMATION:
Tuition Payment
Tuition is due at the time of registration. Please make checks payable to Plugged In. If paying by credit card, please enter credit card information on registration form. Thank you.
Financial Assistance
Partial scholarships are available. Applicants must pay 50% toward the cost of the program. This amount will not be covered by scholarship funds. A minimum payment of $150 must be submitted when scholarship is approved and balance paid in full by end of the session. Please fill out scholarship form (available on the forms page at www.pluggedinband.org) and e-mail completed form to the Scholarship Committee at srizkallah@comcast.net. For questions regarding partial scholarships, please e-mail srizkallah@comcast.net.
